Before Scheduling
Have the following documents available when completing the consent form:
- Insurance card, if applicable
- Medicare card—the red, white, and blue card—for all Medicare beneficiaries
- State-issued identification card
New York State Verification
After scheduling your appointment, complete the New York State COVID-19 Vaccine Form to obtain your eligibility verification.
You must bring the New York State eligibility verification letter to your appointment. Failure to bring it may result in cancellation of the appointment.
